標題[請問] 健保退保申請表 轉出申報表的差別

各位好 最近找到人生第一份工作 到職須知上有寫報到當天需要 "全民健保轉出申報表影本一份" 在健保署看應該是這個 http://goo.gl/WMm417 因此我今天剛剛去區公所辦理轉出 但拿到的是一張黃色單子 叫"第六類保險對象退保申請表" http://goo.gl/vN99mL 公所的阿姨跟我說拿這個給公司就可以了 不太清楚兩者差別 我的猜測是區公所可能是代辦因此是申請表 如果是直接去健保署業務組辦理就可以直接拿到申報表? 所以...我應該再直接去申請一份申報表 還是跟阿姨說的一樣給公司申請表的正 或 影本= ="? -- ※ 發信站: 批踢踢實業坊(ptt.cc), 來自: 220.135.88.230 ※ 文章網址: http://www.ptt.cc/bbs/ask/M.1408099265.A.DC1.html
